Insurance and financial policy

Welcome to Pulmonary & Sleep Specialists! Our physicians and staff look forward to working with you to improve and maintain your health.  We ask that you take a moment to read our financial policies below.  Also, you are encouraged to download and complete the new patient forms and bring them to the appointment.  Patients are asked to present their insurance card and a driver’s license or other photo ID upon check-in each time they are seen for medical services.  Please bring these with you to all appointments.

We collect insurance required co-payments, deductibles and any charges for non-covered services at the time of your appointment. We will file all insurance claims to your insurance company. We accept cash, check, debit card, Visa or MasterCard as methods of payment.  If you are a self-pay patient we will ask you to discuss financial arrangements with our staff at the time the appointment is made.

Returned checks are subject to a handling fee of $25.00.

Medicare Patients

We are participating physicians with Medicare.  As there are now many Medicare plans in effect, we encourage you to familiarize yourself with the benefits of your specific plan and any out of pocket costs you may incur.  Our financial staff will be glad to discuss this with you if you have questions.

Commercial Insurance

Please remember that your insurance policy is a contract between YOU and your insurance company.  If your policy requires a referral from your primary care provider, it is your responsibility to have it with you when you come to the appointment or ensure that it has been sent to us and that it has been received.  Lack of a proper referral may cause your appointment to be re-scheduled.  Our staff will assist with any pre-certifications or prior authorizations for diagnostic procedures or medications that we order for you during the appointment.
